Critical Incident Reporting Scheme
An important development is the of the RCR Breast Group Critical Incident reporting scheme. This is not intended to replace the national scheme for reporting being developed by the National Patient Safety Association, but will enable us to collect data on events specific to breast imaging and hopefully alert units to potential hazards and incidents in a timely fashion. There are several methods by which you can notify us of such incidents.
1. Use the form on the website. Copy it to your managers
2. Download and submit the form on the website by e-mail
3. Send us a copy of the form you fill in for your Trust or the NPSA form.
This will avoid having to duplicate data entry
We will collate the data
which may be submitted anonymously if you wish, and produce a report every
three months. This will be circulated via the Big 18, appear in Network and
be published on the website.
This will obviously evolve as the NSPA scheme gets underway nationally and
we are working with their Radiology representatives. However at present Breast
Group Members have told us what they want is timely feedback and information
rather than learning about errors in the National Press
